Membership Pricing
ADULT MAINTENANCE
For the generally healthy adult who is not on any prescription medications and wants to maintain and optimize their current health.- Includes 4 visits per year.
- If all 4 visits are used, each additional visit is a flat fee of $75.
ADULT RESTORATIVE
For the adult on prescription medications and is committed to restoring and improving their health.- Includes an unlimited # of visits to allow the Member to come in as often as they want to work on their health and ideally come off any prescription medications.
ADULT HORMONE OPTIMIZATION
For the adult who is looking to balance their hormones, which may include Bioidentical Hormone Replacement Therapy.- Includes Adult Restorative membership.
- Includes unlimited # of visits.
- Includes Vitality’s initial comprehensive blood work panel OR one repeat panel for current VFH members. (includes a one-time round of labs only, does NOT include any further repeat lab testing).
- Includes a one-time initial hormone testing panel. (does NOT include repeat hormone testing)
PEDIATRIC ADD-ON
This membership tier is an add-on to any adult membership (0-24yrs). At least one parent/guardian must be a Member.- The first child is $75/month and each additional child is $50/month.
- Includes an unlimited # of visits.
